101-250 EMPLOYEES $  75.00
251-500 EMPLOYEES $100.00
OVER 500 EMPLOYEES $200.00

LESS TWO PERCENT COLLECTION FEE ON ALL EMPLOYEE PREMIUMS

B) $5.00 FOR EACH QUALIFED EVENT LETTER
C) $3.00 FOR EACH CERTIFICATE OF COVERAGE
D) $3.00 FOR EACH INTRODUCTORY COBRA LETTER SENT TO NEW EMPLOYEES
E) IF EMPLOYER REQUESTS QUALIFYING EVENT NOTICES BE SENT BY CERTIFIED MAIL OR CERTIFICATE OF MAILNG, REIMBURSEMENT TO ZAPPA & ASSOCIATES REQUIRED
